Multimedia Policy

I hereby grant Jettlife Youth Flag Football permission and license to use, print, publish, broadcast, or post in Jettlife Youth Flag Football related newsletters, brochures, websites or other media the photographs, video, audio or other materials which I or my child(ren) or ward(s) may have appeared, spoken, written or otherwise participated (“Material”). I authorize representatives of Jettlife Youth Flag Football to use any of this Material without notice, obtaining additional consent or providing additional consideration. This Photography and Multimedia Policy will be applied in conjunction with and consistent with the Jettlife Youth Flag Football Privacy Policy, which is incorporated by reference herein.

I, FOR MYSELF, MY SPOUSE, AND CHILD/WARD, HAVE READ THIS MULTIMEDIA POLICY FULLY AND UNDERSTAND ITS TERMS. I, FOR MYSELF, MY SPOUSE, AND CHILD/WARD AGREE TO BE BOUND BY THIS PHOTOGRAPHY & MULTIMEDIA POLICY, AND JETTLIFE YOUTH FLAG FOOTBALL PRIVACY POLICY WHEN IN THE FUTURE I CLICK ON AN “I AGREE,” “I CONSENT,” OR OTHER SIMILARLY WORDED “BUTTON” OR ENTRY FIELD WITH MY MOUSE, KEYSTROKE, OR OTHER COMPUTER DEVICE, YOUR AGREEMENT OR CONSENT OR WAIVER WILL BE LEGALLY BINDING AND ENFORCEABLE AND THE LEGAL EQUIVALENT OF YOUR HANDWRITTEN SIGNATURE.
